Community Connection Worker

 

Full-time 80 hours biweekly                                                   $35.60-$45.78/hour

 

Position Summary
The Community Connection Worker is an integral member of the Esquimalt Nation Community. This position works within the Health and Programs Departments to provide a range of outreach services to children, youth, and their families. The Community Connection Worker will work in collaboration with the Health Department, the Administration, and the Esquimalt Nation Community to provide crisis intervention to children, youth, and their families, in addition to consulting with program staff to ensure prevention and healthy family development. The Community Connection Worker will provide a wide range of supports to Nation members including referrals to treatment facilities and local resources, assistance with legal challenges and obtaining legal aid, domestic violence and housing crisis supports and support to families dealing with the Ministry of Children and Family Development (MCFD). This position may assist in the development and facilitation of support and prevention groups for adults, children, and youth at Esquimalt Nation, as well as provide on-one-one support to members navigating complex systems.

 

Primary Responsibilities

Support

·       Works within the inter-disciplinary team to build capacity toward developing evidence-based programs and training for communities and related health staff and partners.

·       Provides consultation, education and information to individuals, families, community, and caregivers about external and internal resources.

·       Works to infuse cultural ceremony, tradition, and customs into and around related duties where appropriate and as needed.

·       Collaborates with community health staff, Traditional Healers, families, and community resources as needed.

·       Develops, recommends, and implements practices and procedures to improve quality access to external resources available to community and provides advocacy to address barriers of service.

 

Outreach and Referrals

·       Provides services such as assessment and crisis intervention, consultation with outside agencies, schools, and hospitals.

·       Maintains appropriate client/administrative records.

·       Promotes and maintains effective working relationships with members and families, physicians, schools, and hospitals to coordinate services, secure input, cooperation, resolve conflicts, influence outcomes and encourage healthy interactions between community members and outside resources.

·       Provides support to members who require to obtain legal support and/or legal aid for issues relating to criminal/family matters.

·       Provides support to members who are navigating the Ministry of Children and Family Development (MCFD) and other related government agencies.

·       Evaluates outcomes to ensure effectiveness of services through continuous follow-up plans and actions.

·       Provides referrals to external agencies, internal services, Traditional Healers, and other culturally relevant resources.

·       Provides referrals to treatment centers for members experiencing substance abuse issues, eating disorders, elder abuse or ministry concerns regarding elders, and/or other significant personal challenges.

Training, Education, and Experience
·       BSW or BA in a related field plus 1 years’ experience, or Child and Youth Care or relevant discipline plus 2 years’ experience.

·       Social Services/Social Work diploma plus 2 years’ experience.

·       Minimum of 3 years of full-time experience working with children, youth, and their families, with responsibly for providing a wide range of community based mental health services and/or referrals.

·       Trauma Informed Practice training.

·       Cultural Safety Training.

·       Experience working with Indigenous Communities (preferred).

·       Pursuant to Section 41 of BC Human Rights Code, preference may be given to qualified applicants of Indigenous ancestry (First Nations, Metis, and Inuit), with the required combination of education and experience.

·       Class 5 drivers license and a reliable car.
